Care Connection Rahway
Specializing in short-term rehabilitation, Care Connection Rahway, owned by Alaris Health, is designed to bridge the gap between hospital and home. Our subacute center assists patients recovering from a wide range of conditions, including orthopedic, cardiac, pulmonary, and stroke.
Care Connection Rahwayprovides a smaller, more comfortable setting with a high-ratio of dedicated staff who are trained to assist patients and their families in the recovery process. Whether it’s for a few days or several weeks, our patients have access to our full suite of rehabilitation services seven days a week, including holidays. Our integrated therapeutic rehabilitation programs consistently produce superior outcomes, enabling patients to transition home as safely and smoothly as possible.
Conveniently located on the fourth floor of RWJ Rahway, Care Connection Rahway has access to an array of specialized medical services available, as ordered by the attending physician. Being on the RWJ Rahway campus affords our patients a seamless continuum of care from your acute hospital stay to sub-acute rehabilitation.
Recognizing the uniqueness of each patient, Care Connection Rahway formulates individual rehabilitation plans designed for each client to improve function, rebuild strength, confidence, and achieve the highest level of functioning. The facility integrates a holistic and interdisciplinary team approach depending on the plan care.
Our Services
Care Connection Rahway is a 24-bed facility that truly focuses on positive patient outcomes and customer satisfaction. Our facility is a welcoming and intimate place to heal from an acute hospital stay. In addition to accepting patients from RWJ Rahway, Care Connection Rahway accepts patients in need of sub-acute rehabilitative care from other hospitals.

- Laboratory and radiology services
- Functional electrical stimulation
- IV therapy
- Ultrasound, heat and cryogenic treatments

- Wound care management
In addition to room accommodations, amenities also include complimentary cable television and phone. Patients are able to choose their meals from a restaurant-style menu, in accordance with their specific dietary needs and restrictions.
The Social Services department serves as the client’s advocate and assists in planning for a safe discharge to home and to other community settings. Our licensed social worker also coordinates transfers to extended rehabilitation or long term care facilities including affiliated Alaris Health Centers. Located just one mile from RWJBarnabas Health lies Alaris Health at Riverton, Care Connection's preferred provider for extended sub-acute and skilled nursing services.
