Bridging the gap between hospital and home, subacute care represents a new trend in health care which is the concept of continuing care for the patient who is over the acute phase of illness but still needs medical care and rehabilitation. Designed to provide a homelike atmosphere in a more relaxed environment, the Subacute/Skilled Nursing Facility offers warm, personal care from professional staff members for patients who require:
  • skilled nursing care.
  • daily treatments in accordance with special needs for rehabilitation, including self-care training, medications, dressings, etc.
  • post-operative care and recovery period.
Care Connection has been developed to provide the facilities and trained personnel for an active program of patient rehabilitation and medical treatment. The success of the program depends on the willingness of each resident to handle and participate in their care as their ability allows. Care Connection has an open admission policy with regard to race, religion, color and national origin.

Facilities
Care Connection is located on the fourth floor of Robert Wood Johnson University Hospital at Rahway. A full range of hospital services are provided when needed. Semi-private rooms are equipped with electric beds and closets. Our staff will try to keep you in the same bed and room throughout your stay. For a variety of reasons, however, it may be necessary to move you. We’ll give you as much notice as we can and assist you in selecting another room. Each room has toilet facilities with mirror and sink for the convenience of all patients. Handicap accessible shower and bathing facilities are available. A call bell at each bedside connects to an intercom at the centrally located nursing station, allowing the resident to talk directly to the nurse. A large multipurpose day room is available to be used for dining, recreation, meetings and socialization.

A Typical Day
Your day at Care Connection is designed to increase your independence in daily living activities. You will awake at approximately 7:00 a.m. You will be provided with instruction, training and assistance as needed for bathing, hygiene and dressing. You will dress in your own clothes. After breakfast you will participate in rehabilitation therapies as prescribed by your physician. You may be scheduled in the morning and/or the afternoon. We encourage your family to join you in therapy so they can understand what you are doing and how you are progressing. Recreational therapy is provided individually and in groups throughout the day and on some evenings. Check the weekly activities calendar posted in your room and on the unit. You may have rest periods during the day where you can read, watch TV, socialize. Daily newspapers, magazines and board games are available in the day room. Your family, children, grandchildren and friends may visit you daily from 8:00 a.m. until 8:00 p.m.

Meals
Meals are an important part of the recovery process. Residents are encouraged to eat all meals in the day room with other residents. Menus are selective and appealing. A blank menu for the following day is sent on each breakfast meal tray. Assistance in completing menu selection is provided by Nutrition Technicians on a daily basis. Residents are encouraged to be self-sufficient during meal time. Training, assistance and adaptive equipment for feeding will be provided as needed.

Breakfast 8:00 A.M.

Lunch 12:00 P.M.

Dinner 5:45 P.M.

Guest trays are available for a fee. Meals are also available in Casey’s and the Coffee Shop. Vending machines are located in Casey’s.

A Team Approach
At Care Connection an interdisciplinary team will assist you to become more independent in daily living activities and achieve your health care goals.

The interdisciplinary team will include doctors, nurses, therapists, dietitians, social workers, pharmacists, and other professionals as needed.

All professional staff are graduates of schools, colleges and universities approved by nationally recognized accreditation bodies. Professional staff meet current legal requirements of licensure, registration, certification and competence.

The team will work with you to set goals and design an individualized plan of care to meet your rehabilitative and medical needs.

Team members envision the care of the resident and their family as one process.

The team will meet twice a week to discuss your overall progress and future goals.

The team will work with you and your family to coordinate care and discharge plans.
