Social Services is available to help plan care and services upon a
patient's discharge from the hospital. There is a licensed Social Worker
on each care unit in the hospital. The Social Worker is part of the
healthcare team. Social Workers can link patients and families to
services that can help enhance a patient's function at home and in the
community, such as home-delivered meals, private home care or medical
transportation and hospital van services to medical appointments.
They can make referrals for assisted living, and help with nursing home
placement, or help find outpatient dialysis.
Our Social Workers also can help refer a patient for public assistance,
housing, referrals for legal issues and entitlement programs.
The Social Worker can provide information and referral to assistance
programs such as Medicaid, Social Security Disability, PAAD, charity
care, and other programs.
The Social Worker can help with referrals to short and long-term care
facilities and rehabilition, as well as ambulance transportation.
Social Workers can also help patients fill out an Advance Directive
while the patient is hospitalized and provide educational information.
