About the Diabetes Self Management Program....

This program is designed to meet the special needs of individuals who are living with diabetes - the third most prevalent diagnosis in the nation affecting more than 11 million Americans. It is estimated that approximately 8 million Americans have diabetes and are not yet diagnosed.

Your individualized program will consist of:
  • Stress and psychosocial adjustment to diabetes
  • Use of health care system.s and community resources
  • Nutrition education taught by a Registered Dietician/Diabetes Educator
  • Exercise and activity
  • Proper use of medications
  • Benefits, risks and management options for improving glucose control
  • Monitoring and use of results
  • Relationship among nutrition, exercise, medications and blood glucose levels
  • Prevention, detection and treatment of chronic complications/acute complications
  • Foot, skin and dental care
  • Behavior changes strategies, goal setting, risk factor reduction and problem solving
Who needs the services provided by this program...

Individuals already diagnosed with diabetes Type 1, Type 2, Gestational (a temporary type of diabetes during pregnancy)

People in a high-risk category for developing diabetes.

These individuals:
  • are obese (20% above desirable body weight)
  • have an immediate relative with diabetes
  • are a member of a high-risk ethnic population (African-American, Hispanic, Native American, Asian)
  • have been diagnosed with gestational diabetes
  • have high blood pressure (140/90 or higher)
  • have an HDL cholesterol level 35 mg/dl or lower and/or triglyceride level of 250 mg/dl or higher
  • have ever been told they have impaired glucose tolerance.
About the weight loss programs...

A Lighter Lifestyle Group

The program is an educational series focusing on understanding food labels, the food pyramid, portion control and calorie counting.

The Center also provides an ongoing support group and each session consists of 8 classes which are held Thursday evenings from 5:30 pm to 7:00 pm in Conference Room #3 at the hospital.

Individual Weight Loss Program
  • Individual sessions with a registered dietician
  • First session is one hour; follow-up sessions are 1/2 hour
  • Session may include and focus on:
    • Calorie controlled meal plan
    • Appropriate distribution of carbohydrate, protein, fat
    • Improving nutrient density of diet
    • Improving portion control
    • Improving meal-timing/pattern
    • Behavior modification
    • Vitamin/mineral supplementation
    • Food shopping, health cooking and meal preparation
    • Guidelines for eating out
Exercise
  • Individualized fitness assessment with an exercise physiologist
  • Individualized exercise program consisting of cardiovascular, strength training and flexibility
  • Special program guidelines
  • Monthly re-assessment
