Case Management Department at Robert Wood Johnson Uiversity Hospital at Rahway
While it may sound odd, discharge planning needs to start the day of your admission to the hospital. The goal of discharge planning is to create a smooth transition from hospital to the most appropriate place for your healing, whether that’s at home with some assistance or a rehabilitation or other facility.
The Case Management Department of Robert Wood Johnson University Hospital Rahway consists of registered nurse case coordinators and medical directors dedicated to meeting the needs of patients and families. During an inpatient hospital stay, you will be evaluated by our coordinator to see what kind of care you may need after your hospital stay. The case coordinator also will discuss with you, your family and your physicians what would work best at discharge. We are available for family conferences to help families in facilitating a smooth and timely transition to home or other facility.
We also provide clinical information to your health insurance company for approval of hospital stay days and authorization of ongoing health needs.
