Patient Information

Patient discharge planning starts the day of your admission to the hospital. This allows us to create a smooth transition from hospital to the most appropriate place for your healing, whether that’s at home with some assistance, at a rehabilitation center, or other care facility.

The Case Management Department consists of registered nurse case coordinators and medical directors dedicated to meeting the needs of patients and families. During an inpatient hospital stay, you will be evaluated by our coordinator to see what kind of care you may need after your hospital stay. The case coordinator will also discuss with you, your family and your physicians what would work best at discharge. We are available for family conferences to help in facilitating a smooth and timely transition to your home or other facility.
